    you will wiant one spare wire at each location just in case something happens to a wire you will have a backup for all locations with cat6
    so i would do 
    2 cats to each camera location 
    2 cats to each wap 
    4 cats 1 coax to each tv location 
    2 cats to each lan location 
    2 cats 1 coax to feed from ont/dmark to the main rack 
    2 cats 1 coax from main rack to any location you plan to put another rack or control location
    garage i would always run 4 cats from main rack, depending on how many garage doors you have you will be covered to hardwire some contacts to control your doors 
    1 lutron wire per window covering
    speaker wire
    to properly do speakers anything under 60' but we do 100' will be 16x4 anything over 100' will 14x4 and anything over 180' we will do 12x4
    subs it depends if powered or passive so we will run 14x4 and a shielded rg6
    home theater wiring is always 14x2 to each speaker
    run a 2" conduit from attic to rack area for future needed pulls 
    we also run 4 extra cat, 2 extra speaker, 2 extra coax from attic to rack just in case
    wire is cheap so run extra now
    main tvs can use some extra wire, think about it what if you want to feed audio back to the amp and its a system tv
    1 cat for tv lan
    1 cat if you have local streaming box
    1 coax for cable box
    1 cat for audio feeding back to amp
    1 ir to control streaming box and cable box
    common mistakes made by builders
    uses cheap wire, when securing wires they crush the wire with staples or romex staples, make sure they do not crush the wire, they will also run wire with romex so make sure its at least 14" away from romes on long runs, its ok to cross romex but not run along with it. make sure they dont make 90 degree bends or bend the wire, if the jacket of the wire fades or whitens when making a bend its not good, do not tighten zip ties so it crushes the wire, its not good. honestly i would hire a company to prewire for you, to your requirements as wiring is your foundation and if not properly done could/will cause issues
